To order a profile by mail, please print the following form, complete the form with all information asked for, and send 3 close up photos of yourself. One full face with forehead exposed, and one of each side profile with ear fully exposed. Please tie long hair back close to the head. Make sure the photo is close up. The face should fill the lens with a small border. Also send 6 pieces of hair from each side of your head taken from just above the ear. Tape the hair on a 3 X 5 card and mark left and right.
